Veterinary Forms and Client Handouts Collection
Thanks to the generosity of VINners, the Veterinary Forms Collection contains a variety of sample forms, clip art, and handouts.

Browse the forms database using the categories on the left for:

  • Admission and Drop-off Forms
  • Consent forms
  • Dental forms
  • Health Certificates
  • Maintenance Logs
  • ...and much much more!

Many of the forms can be downloaded as MS Word files that you can modify for use in your practice. More client handouts can be found in the Forms and Handouts section of the VIN Library.

Want to share your favorite forms with other VINners? E-mail the text (or a scanned image) of the form to VINdatabase@vin.com or send a copy of the form via snail mail to 777 W. Covell Blvd., Davis, CA 95616. Be sure to include your name, telephone number and e-mail address with the copy.

With Mobile MyStuff you can access your MyStuff from almost anywhere! Go to your most frequently used document or function with a single click.

The Add to MyStuff for Mobile makes it easy to save quick links to your favorite VIN documents and features while you're on the go.

Click on the red heart on the page you want to save, give your link a new title, or keep the one that pops up, and then click the Save button.

Your link will be automatically added to your Main MyStuff folder, but you can move it to any folder you choose the next time you log in to VIN with your desktop or laptop.

Note: Make sure that pop ups are NOT blocked on your non flash mobile device.

To turn off pop up blocking on your iPad:

  1. Select Settings
  2. Select Safari
  3. Select Off for Block Pop-ups

Granulosa cell tumour in a harbour porpoise (Phocoena phocoena) from German waters
Neoplastic diseases in harbour porpoises Phocoena phocoena have rarely been described, and there are no reported gonadal stromal tumours. A 12-year old female harbour porpoise was stranded on the North Sea coast of Schleswig-Holstein, Germany. Necropsy findings included a severe granulomatous pneumonia, pregnancy and a left ovarian tumour.

The use of IV lipid emulsion for lipophilic drug toxicities
Experimental and anecdotal use of ILE in the human and veterinary literature, theorized mechanisms of action, current dosing recommendations, potential adverse effects, and indications for use in human and veterinary emergency medicine are reviewed herein.

Pharmaceutical evaluation of compounded trilostane products
Compounded trilostane capsules (15 mg, 45 mg, or 100 mg) were purchased from eight pharmacies and assayed for content and dissolution characteristics. Capsules made in-house containing either inert material or 15 mg of the licensed product and proprietary capsules (30 mg and 60 mg) served as controls.

Arachnaphobia
I recently opened a news website to see a picture taken of a spider's multiple eyes... through an otoscope. Yes, it was living in a woman's ear. I may have to have my ears surgically removed now just to make them stop itching and try to get that picture out of my head.
