Help Yourself to the VIN Forms Collection
Thanks to the generosity of VINners, the Veterinary Forms Collection contains a variety of sample forms, clip art, and handouts.

Many of the forms can be downloaded as MS Word files and then modified for use in your practice. More client handouts can be found in the Forms and Handouts section of the VIN Library.

Want to share your favorite forms with other VINners? E-mail the text (or a scanned image) of the form to VINdatabase@vin.com or send a copy of the form via snail mail to 777 W. Covell Blvd., Davis, CA 95616. Be sure to include your name, telephone number and e-mail address with the copy.

VIN Community Quick Polls
Voice your opinion in these one-question polls (some serious, some not so serious)! You'll find a new Quick Poll every Monday on the left side of the front page under the Daily Pearl.

If you missed the previous polls, you can get caught up on those and see results of older polls on the Archived Quick Poll page.

Read the discussion to see how the idea came about and how you can contribute ideas for new polls.
