Wound Management and Reconstruction

The introduction by John Williams touches on the goal of wound reconstruction and basic surgical principles, patient evaluation, and planning management and reconstruction.

In chapter two the reader is taken through classification of wounds and skin deficits, with numerous color illustrations showing the types of wounds, their complications, and their management. Snake and insect stings and bites are discussed, as well as burns, chemical injury, and gunshot wounds.

The third chapter discusses healing and factors influencing recovery. The principles of wound healing, hemostasis, inflammation, and cellular changes are explained and illustrated with drawings and tables.

Wound closure is next, along with pain management, evaluation of wound status and prognosis, level of contamination, clean wounds, antibiotics, wound closure techniques, managing large tissue deficits, wounds associated with orthopedic injury, skin grafts, and degloving injuries.

Without discussing every chapter, I would make the comment that this book is illustrated in such a way as to walk the clinician through managing nearly any type of wound, and describing its healing process. Simple line drawings illustrate surgical steps, healing processes, surgical drains, and suture patterns. Mesh expansion to cover large wounds is illustrated with photos.

Eye and limb wounds are difficult to cover when large areas of skin are missing. Illustrations take the clinician step-by-step through covering these difficult wounds without creating a taut skin closure and further injury.

Some of the wounds shown are horrendous, and one would hope never to see their equivalent come in the door, but you can be prepared for the worst with this book. Skin grafts, harvesting and preparation, and the recipient site are covered in detail. Complications and graft failures are also discussed.

The topics are not restricted to skin injuries; pedicle muscle flaps are illustrated and microsurgery and bone graft techniques are presented in detail. Complications of all of these procedures are examined.

Overall, this book can be a valuable adjunct to the veterinary practice. Don't let this one sit on the shelf until that emergency comes in; read it now and familiarize yourself with its contents so that you will be prepared and know where to look for assistance.
