The presentation to the client should also include a short explanation on any substantial short and long term side effects reasonably expected to result from the vaccination. There is no requirement to inform clients of every possible side effect that could occur.
The required information can be presented to the client verbally or in the form of a prepared handout. The Cornell Feline Health Center's brochure, 'Feline Vaccines: Benefits and Risks', or others with similar content would be appropriate. Although not required, a separate consent form may be used (see Appendix C for examples). In either case, a notation should be made in the patient's record as to what information was provided, by what method it was provided, and the owner's decision as to treatment.
