ANES100-0901: Anesthesia: New Thoughts on Old (and New) Drugs

COURSE DESCRIPTION

COURSE OUTLINE:

Week 1: Discussion of the sedative/tranquilizers used for premedication. The phenothiazines (ie ace promazine), benzodiazepenes (diazepam, midazolam), and the alpha-2 agonists (xylazine, medetomidine) will be covered. Discussion will include mode of action, site(s) of action, effects, side effects, doses and routes. Emphasis will be on when to use each drug, how to select an appropriate dose for the drug, situation and patient, and how to minimize adverse effects from these drugs. Cases will be presented by the instructor and the participants will discuss their premed choices, defend the choice, and discuss their considerations regarding the drug, dose, and route selected.

Week 2: The opioids as an adjunct to the premed (aka neuroleptanalgesia) will be covered. The commonly used opioids (morphine, oxymorphone, hydromorphone, butorphanol, and buprenorphine) will be covered. Discussion will focus on premedication and preemptive analgesia only. A discussion of opioid receptors will be included and will be emphasized. Advantages and disadvantages of the various opioids available to practitioners will be discussed as well as doses and routes of administration. The cases used in week 1 will again be used, this time adding an opioid to the protocol. As in week one, the participants will discuss their opioid choices, defend the choice, and discuss their considerations regarding the drug, dose, and route selected.

Week 3: Induction drugs will be discussed with emphasis on IV induction drugs (thiopental, propofol, ketamine/diazepam, telazol, etomidate). Discussion of why to use an IV induction over mask, chamber or IM will be covered. Mode of action, site of action (if known), advantages, disadvantages, indications and considerations for each drug or combination will be discussed. Using the cases discussed the previous 2 weeks; the participants will discuss their induction choices (based on the premed choice of course), defend the choice, and discuss their considerations regarding the drug, dose, and route selected.

Week 4: The various maintenance options will be discussed, with emphasis on the inhalants. The 3 common inhalants (halothane, isoflurane, sevoflurane) will be discussed. The agents will be compared with discussion focusing on properties affecting the action of each (solubility, vapor pressure, MAC). In addition, discussion of machine kinetics will covered so the participants can get the maximum use of their anesthesia machines, regardless of agent used. In addition, selected intramuscular and intravenous maintenance options will be discussed. Using the cases discussed the previous 3 weeks, the participants will discuss their maintenance choices (based on the premed and induction choices of course), defend the choice, and discuss their considerations regarding the drug, dose, and route selected.

At the end of this course, participants will be better able to:

  1. Understand the various premed options available and tailor their premedication selections to the needs of the patient and the procedure.
  2. Be able to select and mix opioids so as to better provide their patients with adequate preemptive analgesia.
  3. Select an induction technique and tailor it to a variety of patients
  4. Understand the differences between the various inhalant agents and to use them effectively in their practice.
  5. Develop rational, safe balanced anesthetic protocols for a wide variety of patients.

COURSE WITHDRAWAL AND REFUND POLICY: Withdrawal prior to the listed start date of a course entitles the registrant to a complete refund or a credit toward a future VIN CE course, whichever is preferred. Withdrawal within 1 week after the listed start date (i.e. including no more than one real-time session) entitles the registrant to a credit toward any future VIN CE course. (Does not apply to courses with only one real-time session.) After the first real-time session, a registrant may withdraw due to special circumstances and receive prorated credit towards a future VIN course. These requests will be handled on an individual basis. The amount of the prorated credit will be determined based on 65% of the time remaining in the course at the time of withdrawal.
